I think you should look at two things before choosing:
1/ The health care system of the country
2/ The weather (not to warm, otherwise it will be very unconfortable for your wife)

I would suggest the following countries (maybe not exotic, but still interesting to visit):
- Western Europe (France, Italy, UK, scandinavia…)
- Australia can also be a good destination depending on the season. Check the weather first.

    I wouldnt take a pregnant woman anywhere exotic. There is to much risk for her unborn child.

    First I would suggest consulting your docotor about your wife being able to travel. But if I where to suggest anywhere it would be Hawaii. This way you are at least in the U.S. where she and the baby can get good care should they need it. But the flight will likely be too long and again you need to consult a doctor.
